I'll give this a try. Here's my form:

Color Sceme: Hunter Green, Navy Blue and Maroon/Burgundy
Text: Dreaming River Ranch Hunter/Jumpers
Fram Name: Dreaming River Ranch
Animals: Hunter Ponies (only jumping ones, please)
